Case Management

Brochure

Case Management is a process of linking clients to resources in the community to help further their independence. The role of the case manager is to help clients obtain the resources that they need, on an individual basis, to obtain independence and safety.

The Case Manager will help the client help him/herself by working together to create an individual service plan (ISP). This plan will outline exactly what the client would like to accomplish during the time he/she is working with the case manager. This ISP is based on the needs assessment completed within the first three days of service at Project Woman. A client will meet with the case manager on a regular basis to discuss progress on goals and to make any needed changes to the plan. The case manager will provide referrals to different resources to allow the client to find what is needed to accomplish ISP goals.
